The Clerk is the Registrar of Vital Statistics and is responsible for providing many types of certificates and licenses within the Township. Such certificates and licenses are for Birth, Death, Marriage, Civil Union and Domestic Partnership. Since 9/11, the State of New Jersey has put in place numerous regulations and requirements in order for you to obtain a certified copy of a vital record. Proof of relationship in reference to the record you are requesting may be necessary depending on the circumstance. Depending where the birth, marriage, civil union, domestic partnership or death occurred will determine where you need to apply for your vital record. If you have questions, please feel free to contact the Municipal Clerk’s Office at 856-447-3100. If you wish to obtain a vital record, the following stipulations must be met:
FORMS OF IDENTIFICATION
You must include copies of two forms of identification such as :
- Driver’s License
- Social Security Card
- Photo Identification
- Medicaid or Medicare
- Passport
- Voter Registration
- Green card/Immigrant Visa
- Federal or County Id
VITAL RECORD FEES – CERTIFIED COPIES
Birth Certificate
A certified birth certificate that can typically be used for travel, passport, proof of citizenship, social security, driver’s license, school registration, personal identification and other legal purposes. Birth Certificates are available for events that occurred in Downe Township only.
First Copy: $8.00
Additional Copies: $5.00
Marriage Certificate
A certified copy of the marriage certificate. This is not a copy of the marriage license. Marriage license records are available for events that occurred in Downe Township only.

Death Certificate
A certified copy of a death record that can typically be used to obtain death benefits, claim insurance proceeds, notify social security and other legal purposes. Death Certificates are available for events that occurred in Downe Township only.

TO APPLY
- Complete an Application for a Vital Record. You must use this form or your application will not be processed. This form can also be obtained at the Registrar of Vital Statistics office.
- You must provide exact information that answers all the questions and identifies the information as recorded on the vital record.
- You must provide proof of your relationship to the person named on the record. Government photo identification with a current address is required. If you do not have identification you must provide two valid acceptable forms of identification.
- Please pay by cash, check or money order made payable to Township of Downe.
A Local Registrar may issue a certified copy of a vital record only to persons who establish themselves as the subject of the vital record, the subject’s parent, legal guardian or legal representative, spouse, child, grandchild or sibling if of legal age, to a State or Federal agency for official purposes or pursuant to a court order.
VITAL RECORD FEES – MARRIAGE/CIVIL UNION LICENSE
When applying for a Marriage License / Civil Union License if the bride, or groom, is a resident of New Jersey the license must be obtained from the Registrar of Vital Statistics in the municipality where either person lives. If both applicants is a non-resident of New Jersey, applicants must apply in the municipality in the State of New Jersey where the marriage will take place. For marriage license purposes, soldiers are residents of the posts at which they are stationed. A Marriage Certificate may be obtained from the local Registrar in the municipality where the ceremony occurred, or from the Office of the State Registrar of Vital Statistics. TO APPLY
- The application must be applied for in the municipality where either the bride or the groom resides. If both applicants are out of state residents, then the license is applied for in the municipality where the applicants will be getting married. Once the application is issued, you can get married anywhere in the State of New Jersey. The license, once issued, is valid for 30 days.
- There is a three (3) day waiting period from the time the license is applied for prior to issuing same.
- Photo identification with a current address along with a $28.00 FEE will be required at the time of the application.
- A witness over the age of 18 must be present, along with BOTH applicants at the time of the application.
- A certified copy of your marriage certificate may be obtained from the registrar of vital statistics in the boro, township, or city in which your marriage occurred.
- You will use the certified copy of your marriage certificate to change your name with the social security administration and the motor vehicle commission.
